Vintage Celebration Entry Form

To register online, please complete & send the following information. Then mail a check for the $10 per car entry fee by July 21, 2008 to:
Botham Vineyards, 8180 Langberry Rd., Barneveld, Wisconsin 53507
After 7/21/08 the entry fee will be $15 per car.
All registration fees will be donated to the Infant Intensive Care Unit at St. Mary's Hospital, Madison, Wisconsin.

For email entries whose accompanying $10 registration fee has not been received via US postal service by July 21, 2008 please understand that your space reservation cannot be guaranteed and that your day of show registration fee will be $15.

All accepted pre-registered entries will be confirmed by mail no later than July 30, 2008. Entry fees will be returned to those entries that are not accepted. Day of show entries accepted, limited only by space and time restrictions. Make sure you have signed the waiver below before returning this form.

Note: When you arrive at the vintage celebration, you will be required to sign the following waiver:

WAIVER: On acceptance of this entrant for the Vintage Celebration Auto Show, I expressly release all members, officers and agents of Botham Vineyards & Winery and the Vintage Celebration Auto Show from any causes whatsoever that may be suffered and/or claims that may be incurred by myself, my family or my guests while attending said event. I understand that each participant will be solely responsible for their own vehicle and personal property. I state that I have my vehicle insured against personal injury and property damage.
